Back Pain After a Car Accident: Causes and Next Steps

A sudden impact can load the muscles, tendons, ligaments, joints, and discs of the back. NIAMS explains that back pain may involve mechanical or structural problems in these tissues or pressure on a nerve. In a collision, twisting, bracing, seat-belt restraint, or direct contact inside the vehicle may contribute to symptoms.

It is not possible to identify the injured structure from pain intensity alone. Some people have significant soreness without a serious injury, while others have neurological symptoms that require prompt evaluation even when the crash seemed minor.

Tell the clinician whether the pain is sharp, aching, burning, or tight; whether it stays in one place or travels into a buttock or leg; and whether sitting, standing, bending, lifting, coughing, or sleeping changes it. Note any stiffness, spasms, tingling, numbness, or weakness.

Also report prior back injuries, surgeries, osteoporosis, pregnancy, blood-thinning medication, and any treatment received after the crash. Those details can change the examination, imaging decision, or referral plan.

Seek urgent medical care for back pain accompanied by new trouble urinating, loss of bladder or bowel control, weakness or significant numbness in the legs, numbness in the groin or saddle area, fever, severe abdominal or chest pain, or rapidly worsening symptoms. NIAMS specifically recommends medical evaluation for back pain after an injury and for pain accompanied by urinary trouble or leg weakness, pain, or numbness.

Call 911 for severe trauma symptoms, loss of consciousness, breathing difficulty, or inability to move safely. An outpatient chiropractic office is not an emergency department.

The clinician may assess posture, spinal and hip movement, tenderness, strength, sensation, reflexes, walking, and positions that change the pain. Findings that suggest fracture, significant nerve involvement, or another serious condition may lead to imaging or referral before manual care is considered.

For an appropriate musculoskeletal case, treatment may combine education, activity modification, therapeutic exercise, mobility work, soft-tissue care, or spinal manipulation. NCCIH reports that spinal manipulation is one of several options for low-back pain and may produce modest improvements in pain and function for some patients. It is not the right intervention for every injury.

Bring your emergency or urgent-care discharge paperwork, imaging reports, medication list, and a short timeline of symptoms. Include activities you cannot do normally, such as sitting through a work shift, lifting, driving, or sleeping comfortably. Clear functional details make it easier to set measurable care goals.
